REGISTRATION FEE

TO BOTH FOREIGN AND LOCAL PARTICIPANTS, PLEASE TAKE NOTE OF THE FOLLOWING:

• Payment is to be made before 15 July 2009. If we do not receive the payment by then, we will assume that you are not attending the conference and we will not schedule your presentation.
• There is no refund on cancellation after payment has been made. If you are unable to attend after making the payment, you may send a replacement. We will require the name and details of the person replacing you a week prior to the event.

• The conference fee is USD 150 per participant.
• All bank / transfer charges are to be borne by the participant.

• You will be notified once payment is received.

• The following registration fee is applicable:
- RM 400 (members of PPM, ITNM, DBP, USM)
- RM 500 (non-members)
- RM 1,200 (group discount rate - only applies to 3 participants from the same institution)
• Payment can be made by cheque / postal order (Wang Pos) to ‘BENDAHARI USM (PPA12, 2009)”.
• Kindly state your name, institution, contact number, e-mail and ‘PPA12, 2009’ at the back of the cheque.
